Scrotal Reconstruction Using Gluteal Fold V-Y Advancement Fasciocutaneous Flap in Fournier¡¯s Gangrene: A Case Report

Abstract

For successful treatment of Fournier¡¯s gangrene, wide debridement and antibiotics therapy are inevitable. After wide debridement, soft tissue defect of scrotal and perineal area can be a very challenging region for reconstruction by various flap. 39-year-old male had wide scrotal and perineal soft tissue defect and penile shaft skin defect developed by Fournier¡¯s gangrene after dorsal neurectomy for treatment of premature ejaculation. We planned gluteal fold V-Y advancement fasciocutaneous flap for scrotal and perineal soft tissue defect and split-thickness skin graft for penile shaft skin defect. Flap and skin graft survived completely with aesthetically tolerable donor site scar concealed by gluteal fold. we can also preserve sensation similar with donor site. This flap can provide thin, similar color and texture, reliable blood supply, preservation of sensory and aesthetically tolerable scar. And also we can perform easily and closure under a minimal tension, so that ambulation is not limited.
